Similar incidents produced very different records
Client D supported four shopping centres with busy public areas, retailers, contractors and after-hours activity. The centres recorded a high number of operational incidents, but reporting quality depended on the site and the supervisor on duty.
Client managers often received the initial notification quickly, then waited for a complete account. Follow-up actions, such as reviewing footage, contacting a retailer or confirming a maintenance response, were tracked through separate emails. The security team was responding, but the client could not always see the full chain of action.
One incident journey from notification to closure
XION helped Client D define a common incident lifecycle. Reports used a consistent chronology and evidence checklist, with clear severity levels, notification requirements and closure criteria.
The operating model introduced a shared incident classification and escalation matrix, report prompts focused on facts, actions and outcomes, named ownership for follow-up tasks, supervisor review before client closure, and portfolio reporting for recurring locations, times and incident types.
Better information changed the response
In this illustrative story, incomplete incident reports reduced from 26% to 6%. Median acknowledgement of high-priority escalations improved from 12 minutes to 4 minutes, and actions open for more than seven days fell from 19 to 4.
Client discussions became more constructive because both parties were working from the same operational record. Client D could identify patterns across centres and focus supervisor attention where it would have the greatest effect.
